01 The problem

Where PMHNP claims usually go wrong

Psychiatric coding is specific enough that general billing misses things. Scroll to watch one claim get checked, point by point, before it goes out.

01 / 06

Psychotherapy add-ons

90833 90836 90838 denied or paid wrong when they go out with an E/M.

How I fix itI check every E/M + add-on pair before it goes out, and appeal the ones paid wrong.
02 / 06

Evaluations

90791 90792 coded in a way the payer rejects.

How I fix itI code evaluations the way each payer expects, and correct and resubmit rejections.
03 / 06

Enrollment & scope

NPs denied for enrollment, credentialing or scope-of-practice reasons.

How I fix itI keep NP enrollment and payer rules in mind on every claim, and can help with enrollment.
04 / 06

Telehealth

Claims bounced over modifiers or place of service.

How I fix itI check each payer’s current modifier and place-of-service rules before the claim goes out.
05 / 06

Prior authorizations

Prior auths that nobody started early enough.

How I fix itI check what needs authorization before the visit and track approvals so nothing expires.
06 / 06

Aging A/R

Old claims in A/R that nobody followed up on.

How I fix itI follow up with payers on older unpaid claims so money doesn’t sit stuck.

Most of this is fixable. It just takes someone checking claims before they go out and chasing them after.

04 About me

A biller who focuses on psychiatry.

I’ve been an independent biller for 2+ years, working with US providers on claims, denials and A/R. I’ve worked in Kareo/Tebra, SimplePractice and TherapyNotes, and sent claims through Availity and Jopari. If your practice uses something else, I’ll learn it.

I focus on psychiatry because the coding is specific enough that general billing misses things.

When you message me, you get me, the same person every time.

05 Working with someone in Pakistan

Offshore, but on your clock.

I’ll say it upfront: I’m in Pakistan, and I work US hours so you’re not waiting a day for a reply.

Before we start, we’ll check your payer contracts to make sure offshore access is allowed. Some payers restrict it, and I’d rather find that out in week one than in month three.

01What does a psychiatric billing service actually do?
I take care of the money side of your practice: submitting claims, posting payments, following up on denials and unpaid claims, and checking eligibility. You focus on patients, and I make sure the claims behind those visits get paid correctly.
02Which CPT codes do you work with?
Mainly the psychiatric codes: 90791 and 90792 for evaluations, E/M codes for medication management, and the psychotherapy add-ons (90833, 90836, 90838). I also handle telehealth claims. If your coding has a question I’m not sure about, I’ll flag it rather than guess.
03Do you bill for PMHNPs?
Yes, that’s my main focus. NPs run into their own issues with enrollment, credentialing and payer rules, and I keep those in mind when I check claims.
04How do you handle prior authorizations?
I check whether a service needs authorization before the visit, send the request, follow up with the payer and track the approval so nothing expires unnoticed.
05Can you bill telehealth sessions?
Yes. I check each payer’s current rules for modifiers and place of service, since these differ by payer and change often, before the claim goes out.
06What happens when a claim is denied?
I find the reason, correct the error and resubmit or appeal. If the same denial keeps coming up, I tell you what’s causing it so it stops happening.
07Will outsourcing save me money compared to hiring in-house?
Often it can, because you skip salary, training and software costs for a biller. Whether it makes sense for you depends on your volume and payer mix. After a free review I’ll give you an honest view, even if the answer is that it isn’t worth it yet.
08Do you work with practices that already have billing software?
Yes. I’ve worked in Kareo/Tebra, SimplePractice and TherapyNotes, and have sent claims through Availity and Jopari. If you use something else, I’ll learn it.
09How do you protect patient information?
I sign a BAA before I see any patient information. I only access what billing needs, and I use my own login (never a shared one). I don’t keep patient data anywhere else.
10How quickly can we start?
It depends on how quickly I get access to your system and what your current setup looks like. We start with a free review, then a 30-day pilot if it makes sense.
11Do you handle credentialing?
Credentialing isn’t my main service, but I can help when it’s part of what you need. That can include gathering the required information, preparing and submitting payer enrollment applications, and following up on their status. Since enrollment affects how and when your claims get paid, I’m happy to make sure it’s set up correctly. Tell me what you need during the free review and we’ll go from there.
12What reports will I get?
A simple monthly summary: what was billed, what was paid, what was denied, and what’s still sitting in A/R. No confusing dashboards, and I’m happy to walk you through it on a call.
